I  Am 21 +  years, whose address and identity have been certified by the attached photocopies.
I hereby confirm and assure that I am adopting the pet with wholehearted consent of my family, they
are all aware of the responsibilities and challenges of having a pet at home. I am adopting the pet for my
own joy and not for gifting, he/she shall neither be sold or exchanged for any consideration, nor transferred
to or left with any person who may not comply with the conditions and requisites mentioned here. He / She
is being adopted only for companionship and not for the purposes of breeding, entertainment, religious
rituals or consumption. I am adopting the said pet fully aware of my obligations as a pet parent and shall
ensure that I provide him / her all the attention, health care, love, exercise and amenities as required
and advised, and have him / her regularly checked by a qualified veterinarian and vaccinated.
I also promise that I shall remain a responsible parent and guardian throughout his / her life time, and at no
point in time shall I return, disown, abandon, transfer, put down, neglect or disrespect his / her rights as
defined by law and as necessary as a socially responsible citizen. I understand that taking home a pet is a
responsibility for life and that I shall under no circumstances abandon my pet, leave him / her uncared for
or with children without adult supervision or expose him / her to any kind of danger. I shall not keep him /
her chained, confined in unhealthy conditions, exposed to the tempers of nature like extreme, heat, cold or
rain, and away from hazardous materials / areas like fire crackers, gensets, motors, pump rooms, sump, wells
etc.
I am aware that animal cruelty is an offence – under Section 11 of the Prevention of Cruelty to Animals Act
and Section 428 of the Indian Penal Code, punishable with imprisonment and fine and that any ill treatment,
abuse, neglect and any other acts that endanger the rights of the pet, or can cause him / her physical harm
or mental trauma will all come under animal cruelty and I can be held responsible. I have been advised, and
accept that Section 3 of the Prevention of Cruelty to Animals Act, 1960, casts upon me an obligation that I
do the same and more, for the pet that I’m adopting and that will be in my care.
I understand the fostering parent or any person acting on their behalf have the right to examine the pet any
time, checks its state of health, status of vaccinations, his / her living conditions, care taken and take action
as necessary at that time under advice to me.
